How do I fill out the Intake Form?
To complete your intake form, follow the instructions below. The portal automatically saves your progress, so you can leave and return to complete it as needed.
For U.S. Customers:
- Visit https://portal.directmeds.com/
- Enter your account email and password
- Click Sign In to access your dashboard
- The intake form will often load automatically. If it doesn’t, click Complete Intake on the top of your dashboard
- Follow the instructions to provide the necessary health information and upload any required documents, images, or videos
- Submit the form to finalize your intake
For Canadian Customers:
- Visit https://caportal.direct-meds.com/user
- Enter your account email and password
- Click Sign In to access your dashboard
- Click Complete Intake on the notification at the top of your dashboard
- Follow the instructions to provide the necessary health information and upload any required documents, images, or videos
- Submit the form to finalize your intake
