How do I fill out the Intake Form?
To complete your intake form, follow the instructions below.
For customers located in the United States:
Enter your account email and password
Click "Sign in" to be directed to your dashboard
Click "Complete Intake" on the notification at the top of your dashboard
Follow the instructions to provide the necessary health information and upload any necessary documents, images, or videos
Be sure to submit the form to finalize your intake
For customers located in Canada:
Enter your account email and password
Click "Sign in" to be directed to your dashboard
Click "Complete Intake" on the notification at the top of your dashboard
Follow the instructions to provide the necessary health information and upload any necessary documents, images, or videos
Be sure to submit the form to finalize your intake.
The portal automatically saves your progress as your progress throughout, allowing you to leave and return to complete it as needed.
