United States Patients

  1. Go to https://portal.directmeds.com/
  2. Log in with your account email and password
  3. On your dashboard, click “Complete Intake” (it often appears as a notification at the top)
  4. Provide the requested health information and upload any required documents, images, or videos
  5. Click Submit to finalize your intake

Canada Patients

  2. Log in with your account email and password
  3. On your dashboard, click “Complete Intake” (it often appears as a notification at the top)
